[coyotos-dev] SMP Patch 1

Hello,

The attached patch reflects the minimum number of changes I needed to
make after merging with Coyotos rev 285. With this patch we can once
again boot SMP.

The most notable change is 32-bit address use in the 16-bit part of
the AP bootstrap page. AP bootstrap pages can now be loaded above 64k
(and below 1mb) as intended.

We also catch up with changes from CPU.stack to CPU.topOfStack.

Removed a little from the per-CPU kernel map, but not all.

The next patch will incorporate Shap's emailed suggestions, shrinking
things quite a bit.
