Hello,

Attached is my patch against changeset 278 aka b68e1214e2d2. Warning:
very rough.

- does not handle kmap growth after BSP init
- does not do any CPU-CPU IPIs for TLB invalidation
- no timers (or any IRQs) are enabled/handled on the APs (so APs do
not preempt their occupying processes)
- PAE only (do we care about SMP on non PAE?)
- no CPU identification
- races in AP startup (only one AP can boot of a boostrap page at a
time, we rely on luck and races when cpu_ncpu > 1)

That's it.
